Person-centered care is a fundamental approach to providing support and care to individuals that prioritizes their needs, preferences, and values. Attendants working with ICFDDN residents must understand and implement this approach effectively.
What is Person-Centered Care?
Person-centered care ensures that the individual receiving support is at the core of every decision and interaction. This approach goes beyond fulfilling paperwork requirements. It involves understanding the person’s unique identity, preferences, and aspirations, as well as including their family and support network in the process. The primary focus is on the individual, ensuring their voice is heard and respected. Attendants are expected to:
-
Ensure the person being supported is the central figure in all discussions, planning, and decision-making processes.
-
Develop care plans and strategies that prioritize the individual’s needs, preferences, and aspirations.
- Empower individuals to actively participate in service planning by choosing the time, location, and agenda of discussions.
-
Engage directly with individuals to understand their personal goals, family dynamics, and life objectives. Learn about individuals’ hobbies, interests, values, and cultural preferences to respect their unique identities.
-
Encourage individuals to lead conversations, share their ideas, and express their preferences openly.
-
Adapt communication styles—verbal, non-verbal, or gestural—to ensure the individual feels understood and heard. Listen attentively to individuals’ words, actions, and behaviors while understanding input from those who know and care about them.
-
Take a holistic approach to care, considering the individual’s healthcare needs, home environment, and social connections.
-
Provide care tailored to the individual’s physical, emotional, and social requirements, ensuring it aligns with their dignity and independence.
-
Support individuals in performing tasks independently, offering assistance only when necessary, to promote their autonomy.
-
Build trust and foster positive relationships through empathy, professionalism, and respect in all interactions.
